Living With Change™ is an education and preparedness-focused organization dedicated to helping people living with dementia, along with the families and organizations that support them, prepare thoughtfully for the practical realities of life beyond medical care.

Our work centers on education and early preparation, not consulting or clinical guidance. We translate complex and often overwhelming information into structured guidance that helps people prepare for life-impacting decisions and conversations, supported by thoughtfully designed tools that can be used before meeting with attorneys, healthcare providers, financial advisors, and other professionals.

Living With Change complements medical care by focusing on the practical planning and coordination that extend beyond the clinical setting and into everyday life.

Founder & Leadership Grace A. D. Walfall, MBA, CDP®

Grace brings more than 25 years of experience in healthcare operations, public health, and nonprofit leadership. As a Certified Dementia Practitioner and Independent Trainer and Consultant with Teepa Snow’s Positive Approach to Care®, she combines a deep understanding of dementia with a strong operational lens to help organizations and families reduce unnecessary complexity, strengthen communication, and implement practical preparedness systems that support daily life.

Her work is informed by decades of leadership in operational transformation, community engagement, and person-centered care. Through Living With Change™, Grace applies this experience to developing education, frameworks, and tools that help individuals, families, and care organizations prepare for the non-clinical realities of dementia in a way that is sustainable, dignified, and grounded in real-world practice.
